- Troop finance reports for 1 October 2010 - 30 September 2011 are due between June and September 2011. Finance reports through 30 September 2010 are overdue. Please attach a copy of the troop's latest bank statement (matching the report).
- Troop/Group Activity Application (TGAA) - We need them in 2 weeks in advance, for anything besides LINK programs and the troop's "regular" troop meeting times & places. They *are* required for troop booth sales.
- Permission slips
The "Troop Trip" is really a "Troop/Group Activity Application" TGAA form. Fill out the front for day trips and attach a roster. For overnight trips fill out both sides. The Service Unit needs a copy - at least 2 weeks beforehand, including for high-risk or overnight. If you want a copy back, give us two. For high risk & overnight we need 3 copies (1 for GSNNJ, 1 of us & 1 to return to you).

Health forms: My understanding is that due to the HIPPAA confidentiality laws, we can not require a Health History form for basic meetings. There is a Girl Needs Assessment form to collect information that you may need. My personal opinion is to encourage people to share allergy information so that everyone can be safer. Q: What do I need to do when non-GS come to an event? Not all parents and siblings are registered Girl Scouts and I do want to invite them to our Court of Awards; and we want "fathers"/significant adult males in their life to our "Me and My Guy" events; and the last "bring a friend" meeting was such a success!
A: You need to fill in a "Plan 2" form to request insurance for them. It is $5 for the first 40 people per day - whether your event is one hour or 24 hours long. The form needs to get to GSNNJ before the event; they send it on to the insurance company, not us.
Q: Where is the Transportation Form?
A: There is no Transportation Form. The information required is collected on the back of the Application for Volunteer Position.
Q: If a parent is a registered Girl Scout, can they drive other girls in the troop?
A: No, registering is not enough to work with the girls or handle their money. parents (& other adults) also need to complete the Application for Volunteer Position. (This should not be be confused with the Leader/Co-Leader Agreement Form which is an annual renewal. If someone is only interested in working at meetings &/or being a treasurer, they might fill in most of the application but not necessarily the Automobile Information box on the back. There is not a unique "Transportation Form", just that box on the Volunteer Application.
The Volunteer Application is required for any leaders/coleaders/treasurers/drivers that have (initially) registered since 1 October 2007. If someone registered before then AND filled out the appropriate council (MAGSC or Lenni or ...) form, they are "grandfathered" in and covered and do not need to fill out the new form. There have long been Volunteer Applications required from council, but before 10/1/07, MAGSC did not require a Social Security Number. While staff at GSNNJ could go through years and years of boxes of paperwork from previous years, that information if not online & it is "on you honor" to remember whether you filled it in. (I know that we gave the form to the parents in my troop because I remember the discussions about giving out their insurance information & license numbers. )
The form is sent to the Paramus office, we do not see it & never touch their SSN. If there is a problem with the Volunteer Application, Paramus office would let Mary Ann and I know in about a week. Otherwise, we would see that someone "passed" in our (about) monthly records. In the file of GS training records that I have from GSNNJ, the first 2 columns are "Vol App'l" and "Transport" so I can always check names for you and see if it was completed (since 10/1/2007).
To transport girls, you must carry a minimum insurance of $100,000/$300,000, operate a registered vehicle, and have a valid Driver’s License.
